Letters and consent forms were mailed to Tomahawk High School and Tomahawk Middle School students’ parent(s)/guardian(s) and forms were stuffed in Tomahawk Elementary School students’ backpacks.

Flu shot consent forms were made available to MAPS students through Skyward and completed electronically during the school registration process.
